I, _________________, agree to be a foster parent and care for a cat or cats belonging to THE FORGOTTEN under the following conditions:

I understand that officers of the THE FORGOTTEN, retain legal ownership of said cat(s) while it is in my care.

I agree to isolate any incoming cat, including this cat, from all other cats not with this cat for a minimum period of 4 weeks or longer, depending on the results of veterinary checks.  I understand the need for isolation to protect my own cats and I do have the ability to isolate this cat from all others.

I understand that THE FORGOTTEN holds final authority over all decisions concerning this cat including medical treatment, foster home placement, euthanasia, and adoption.  I agree to respect any decision that THE FORGOTTEN makes concerning this cat and to help execute these decisions when I am asked and able to do so.

THE FORGOTTEN will be responsible for all veterinary costs while the cat is in your care.  I understand that THE FORGOTTEN must be notified before taking cat to vet to verify reasons and cost.

I agree to provide appropriate, high quality food and water for this cat in a healthy and sanitary manner.  I will ensure that this cat has access to these things at all appropriate times.

I agree to provide a litter box with cat litter.  I will scoop daily and regularly clean and disinfect this litter box to maintain a healthy environment with minimal odor.  I also agree to clean any litter box materials (litter, urine, etc) in a timely fashion should these materials appear outside the box.

I agree to treat this cat in a kind and respectful manner, encouraging good behavior and confidence in order to create a more well-adjusted, adoptable cat.

I agree to comb this cat and clean the eyes daily. I will wash this cat every 7-10 days except when medically contraindicated or unless a different schedule of bathing is approved by THE FORGOTTEN.

I agree to maintain a healthy level of cleanliness for this cat and his surroundings.

I agree to make myself reasonably available to prospective parents for this cat.  I agree to set up times for prospective adopters to meet this cat when they expressing interest.  I agree to discuss this cat with prospective adopters in a very honest and forthright manner.  I agree that THE FORGOTTEN will have the final say in all placement arrangements and I may not place a cat without their approval.

I understand that while THE FORGOTTEN may make recommendations concerning how to deal with this cat on a daily basis, including quarantine, training, etc, these things are under my control and not their direct control.  I therefore take full responsibility for any damage this cat does to my living area, pets, belongings, or myself with the understanding that things occurred because I did not properly protect my living space, pets, belongings, or myself from this cat.

I understand that negligence of any kind regarding this cat, particularly negligence which results in injury to this cat will be dealt with on an individual basis.  I will comply with any decision THE FORGOTTEN makes concerning negligence, whether it be removal of this cat from my care, or asking me to pay all or some veterinary fees incurred, or refusing to allow me to foster any more THE FORGOTTEN cats , or combination thereof.

I agree to notify THE FORGOTTEN immediately of any kind of problem concerning this cat that arises.

I agree to administer proper medications, etc, as prescribed by this cat's veterinarian.  I agree to abide by any other recommendations made by THE FORGOTTEN concerning medical issues.

I understand that I may not be reimbursed for expenses such as food and litter.

I agree to communicate regularly with THE FORGOTTEN concerning this cat.  I agree to notify THE FORGOTTEN if I am planning for a vacation or move at least three weeks in advance so that they can decide what to do with this cat at that time.

I certify that by taking this cat I and my family will not be in violation of any local legislation or numbers limits.

I certify the above to be true and complete to the best of my knowledge.
